Individual Support Services

Services and activities are identified together with each person and their planning team. Not every service applies to every individual — the list below describes what can be part of a plan.

  • Personal assistance with daily living tasks
  • Health care monitoring and behavioral support
  • Socialization and community integration
  • Recreational activities, outings, and special events
  • Occupational, physical, and speech therapy (as needed)
  • Nutritional support, including dance and stepping activities

What each area of support involves

Plain-language descriptions, so families and planning teams can see exactly what a day at Innovative Center can include.

Individual Support

Everything starts with the person. Support is planned around individual needs, goals and abilities — not a fixed schedule everyone has to fit into.

  • Personal assistance with daily living

    Respectful, hands-on help with the everyday tasks that make up a day, provided at the level of assistance each person prefers and requires.

  • Person-centered planning

    Services and activities are identified together with the individual and their planning team, and adjusted as goals change.

  • Counseling and guidance

    Encouragement, problem-solving and a steady adult to talk to when something is difficult.

Health & Wellness

Wellness support during program hours, coordinated with the people who already know each individual best.

  • Health care monitoring

    Attentive monitoring during the program day, with families and the planning team kept informed.

  • Behavioral support

    Consistent, dignified support strategies, applied as identified by the individual's planning team.

  • Nutritional support

    Encouraging healthy habits through the program day, including active options such as dance and stepping.

Therapy & Specialized Support

Additional services can be part of a person's day when they are identified by the planning team.

  • Occupational, physical and speech therapy — as needed

    Available as needed and when included in an individual's plan. Innovative Center coordinates with the providers and team members involved.

  • Other services identified by the planning team

    If the planning team identifies a support that would help someone reach their goals, it becomes part of the conversation.

Community Integration

Being part of the community means being out in it — regularly, and with the support to enjoy it.

  • Community outings

    Planned trips that build familiarity and confidence in everyday public places.

  • Socialization

    Daily opportunities to spend time with friends, share a table, and belong to a group.

  • Special events

    Celebrations and seasonal events that give the year its landmarks.

Daily Living Skills

Practical skills, practiced in a supportive setting, that carry over into life at home.

  • Everyday routines

    Working step by step on the tasks that make independence possible, at a pace that feels achievable.

  • Adaptive equipment use

    Support in using adaptive equipment comfortably and confidently.

Learning & Recreation

A day should include things worth looking forward to. Recreation is not filler — it is where a lot of the growth happens.

  • Recreational activities

    Fitness, games, music, art and movement, offered throughout the week.

  • Educational activities

    Reading, learning and technology, matched to individual interests and abilities.
